Plan My Care is being developed in partnership with Health Launchpad, a joint initiative between the Young Foundation and NESTA. Health Launchpad's aim is to accelerate innovation in healthcare. We are an early stage social venture fund developing promising ideas into new ventures in the field of long term conditions. We provide intensive hands-on support in the form of financial, intellectual and social capital.
