Plan My Care provides online information and tools to simplify the journey for individuals whilst providing Local Government with the overview required to fulfil statutory obligations.
Plan My Care offers unique market access to both state funded and self funded individuals for providers of care services.
Needs assessment - entered directly into Plan My Care or imported from a care management package. The needs assessment can either be self assessed or performed by a care broker or manager.
Develop support plan – completed by the personal budget holder or the care broker.
Agree support plan – the care manager and the individual or care broker can work collaboratively on the support plan which can then be approved online.
Profile the personal budget – the personal budget and any additional income streams can be profiled to create an overall budget that can be tracked and managed.
Procure services – a marketplace providing access to social care providers. The marketplace utilises best practice procurement tools such as reverse auctions and lastminute.com functionality.
Financial management – goods and services bought directly through the marketplace are reconciled against the profiled budget. Records for goods and services purchased outside of the marketplace can be entered manually. Electronic PCard statements can be imported.
Provide feedback – open and transparent feedback and quality assurance of the goods and services delivered by providers. Personal budget holders or care brokers will be able to provide feedback and rate the experience.
Review needs and support plan – all involved parties can collaboratively review and update the needs assessment and support plan online. Any changes are reflected in Plan My Care and the lifecycle of the above steps repeated.
